Submission Timeframe

Please fill out the following form completely.  Requests should be submitted at a minimum of 45 days in advance of the event date to fulfill staffing needs.  Requests received less than 45 days from an event can still be submitted; however, due to contractual requirements for scheduling procedures, there are no guarantees of adequate UPPS staffing for your event. If you do not fill in all the information, your request will not be able to be processed.  Once your event request is submitted you will receive a confirmation email your request was successfully submitted.  Please use the reference number on the confirmation email if further inquires with our special events staff are required.
